Volunteer registration form

The undersigned, for themselves and their heirs, and assigns, do hereby consent to the participation as player or otherwise, by their son or daughter (identified above) in the activities of the ST. MAARTEN/ST. MARTIN BABE RUTH BASEBALL SOFTBALL ASSOCIATION and do hereby jointly and severally release the managers and assistant managers (hereinafter referred to as Staff), and anyone acting with or for said Staff, including League officials and League sponsors, from any and all claims of any nature (including, without limitation, claims for medical expenses by reason of injury to their child) which may arise out of or in connection with the operation, sponsorship, or conduct by the said Staff of the ST. MAARTEN/ST. MARTIN BABE RUTH BASEBALL SOFTBALL ASSOCIATION, and do hereby, jointly and severally, agree to indemnify the said Staff and anyone acting with or for said Staff, against and save them harmless from any and all claims for personal injuries or damage to property which their child, or parent, guardian, next friend or other representative of their child, may at any time assert against the said Staff, or others acting with or for them in connection with the operation, sponsorship, or conduct of the ST. MAARTEN/ST. MARTIN BABE RUTH BASEBALL SOFTBALL ASSOCIATION.
